The Dr. George C. Simmons Counseling and Support Center provides the following programs:

Chemical Dependency Outpatient Treatment for 18 years and older
Developmental Disabilities Service Coordination and Family Reimbursement
Ryan White Title II Program

An Assessment and Intake Evaluation are required for each individual seeking services.  The evaluation takes approximately one and a half hours.  Two photo IDs are required at intake interview in addition to a one time $100 evaluation fee if Medicaid is not active.   All services adhere to strict confidentiality.

Our comprehensive services include:

  • Individual counseling

  • Group counseling

  • Education and orientation to self-help groups

  • Alcohol & Substance Abuse disease awareness and relapse prevention

  • Education, risk assessment & supportive counseling

  • Social and health care services

  • Skill development in accessing community services, information and education about nutrition, vocational and educational services

  • HIV case management, support services, and referral

  • Service coordination

What's at the Counseling & Support Center?

Mission:  To maintain an environment that fosters positive choices and alternatives for improved        quality of life through the delivery of comprehensive, integrated, and user-friendly services.

Dr. George C. Simmons Counseling & Support Center is located at 585 Joseph Avenue in the Northeast Sector of the city of Rochester, New York. We are licensed by the New York State Office of Alcohol Substance Abuse Services (OASAS), New York State Department of Health AIDS Institute; New York State Office of Mental Retardation and Developmental Disabilities through the Finger Lakes Developmental Disabilities Service Office (FLDDSO).

We provide a variety of comprehensive treatment services to inner city residents of Rochester (men and woman 18 years and over) who have a history of Substance Abuse and/or have on-going problem with substance use. Our service continuum includes Intensive Case Management for adolescents, the HIV positive and PWA's; Comprehensive Case Management and Service Coordination for individuals and families with Developmental Disabilities; and targeted outreach activities designed to provide age appropriate HIV education and prevention services to high risk population within the City of Rochester.

Our general philosophy has been to create an environment that promotes wellness and recovery and within which AOD dependent persons and their families, the HIV infected, and the developmentally disabled are empowered to attain and maintain sustained sobriety, abstinence, integration, productivity, and self-sufficiency.

The program provides treatment service procedures to our client population that ensures high level of care, culturally sensitive service environment, and quality care.

Developmental Disabilities

Developmental Disabilities are a variety of conditions that become apparent during childhood and cause mental or physical limitations. These conditions include autism, cerebral palsy, epilepsy, mental retardation, learning disabilities, and other neurological impairments.

Baden Street Settlement is dedicated to helping people of all ages with Developmental Disabilities and their families obtain the support and service they need and desire such as:

  • Counseling

  • Family Support

  • Social Adjustment Skills

  • Employment Preparation

  • Recreation

  • Service Coordination

Case Management
Connecting an individual with Developmental Disabilities to the services they desire begins with a professional Service Coordinator/Case Manager, who provides assessment, services planning and referrals to the many service options available.

Partners in Service
Developmental Disabilities service program is funded by a grant from the New York State Office of Mental Retardation and Developmental Disabilities, through its local unit, Finger Lake Developmental Disabilities Service Office.

Ryan White Title II Program

Intensive, confidential, and professional case management services are provided to all clients. Case Management's goal is to link clients with a full range of services available in the community.

On site services include:

  • Case Management

  • Counseling

  • Health Risk Assessment

  • TB Testing (off-site)

  • Crisis Intervention

  • Emergency Referrals for Food, Clothing, Housing, and Medical Care

  • Advocacy

Eligibility:
Any individual who is HIV positive, PWA, their families, and significant other or individuals that feel they are at high risk of contracting HIV may avail themselves of our services, which adheres to strict confidentiality rules.

AIDS services seek to empower people living with HIV/AIDS and their families. Improved quality of life is achieved by providing intensive case management and assistance for clients and their families. HIV/AIDS` services seek to keep families together by promoting and accessing services, which encourages self-sufficiency.

There are no fees for our services.

Ryan White Title II Program is funded by New York State Department of Health, AIDS Institute.
